Root Cause Analysis (RCA) Resources
ABS Consulting provides you with free resources to perform your incident investigations and root cause analyses. Used in conjunction with our SOURCE™ methodology and our Root Cause Analysis and Incident Investigation Training Course, these tools will help you perform efficient and effective analyses.
SOURCE Investigator's Toolkit™
ABS Consulting’s SOURCE™ Toolkit is designed to assist an investigator during an incident investigation or root cause analysis. The Toolkit is filled with useful forms for collecting and organizing data, as well as useful checklists to ensure nothing is overlooked or missed. It also includes an Incident Investigation Report Template. Download a portion of the Toolkit in Adobe Acrobat format.
Timeline, Cause and Effect Tree and Causal Factor Chart Template
Download our FREE Microsoft® Excel™ worksheet template. Using this template allows you to document your timelines, cause and effect trees, and causal factor charts. There is no special software to purchase, install, or learn. Documenting your analyses is a simple, straightforward process that can be performed on any computer that has Excel installed. It also allows you to easily e-mail the document to others in your company; they don’t need a special program to view or comment on your analysis. And, your analyses can be easily inserted into a Microsoft Word document. Step-by-step guidelines on how to develop timelines, cause and effect trees, and causal factor charts are included in ABS Consulting's Root Cause Analysis/Incident Investigation training course.
